Orto-Perio Risk Assessment (OPRA) in Adults and Adolescents

The interaction between Orthodontics and Periodontics creates a powerful synergy that enhances treatment outcomes for both adults and children. Orthodontic therapy can significantly support periodontal treatment by improving its predictability, expanding therapeutic possibilities, and facilitating surgical interventions. In this context, orthodontics should be regarded as an integral component of periodontal therapy. Conversely, periodontal treatment enhances the effectiveness and long-term predictability of orthodontic care, resulting in a truly interdisciplinary approach. Successful management of ortho-perio patients depends on comprehensive diagnosis, meticulous treatment planning, and well-defined clinical protocols. These principles can be applied across vestibular, lingual, and clear aligner techniques through an integrated clinical workflow that combines risk assessment, appropriate treatment timing, and biomechanical principles specifically adapted to periodontal patients. Ortho-Perio Risk Assessment (OPRA) in adult patients with established ortho-perio treatment necessities and early diagnosis, even in adolescent patients through the p-OPRA (pediatric-Ortho-Perio-Risk Assessment) protocol, allows for the early interception of risk factors for periodontal disease and mucogingival problems and the achievement of long-term stability. The synergy between the two specialties offers an ortho-perio timing to standardize a work protocol and relationship between the different disciplines with mutual and daily updating and cultural sharing among clinicians, so that tradition marries new technologies.
